- Eligibility Check: Ensure you meet the eligibility criteria for financial assistance. This typically includes income verification and proof of medical necessity.
- Gather Required Documents: Prepare the necessary documentation, including Proof of income (e.g., recent pay stubs, tax returns)
- Medical records or a letter from your healthcare provider indicating the need for the biopsy or MRI.
- Insurance information.
- Complete the Application Form: Fill out the financial assistance application form available on our website. Make sure to provide accurate and complete information.
- Submit Your Application: Send your completed application form and supporting documents to Biopsy Inc. via email at Biopsyinc@Biopsyinc.org or mail to Biopsy Inc. P.O. Box 144 Germantown, Maryland 20874
- Await Confirmation: Once your application is received, our team will review it and contact you with the next steps. If your application is approved, you will be notified of the payment date, and the payment will be sent directly to the facility or office requiring the co-payment on your behalf.
